The following enrollment forms are necessary to complete the enrollment process:
- Registration Confirmation
- Enrollment Agreement
- Student Vital Information Form
- Primary Parent Vital Information Form
- Co-Parent Vital Information Form (where necessary)
- Grandparent Vital Information Form
The enrollment process can be completed online. Enrollment forms must be completed and submitted by the applicant's parents/guardians by the date indicated in your admission notification email. F-1 Student Visa for International Students
International students who need the F-1 Student Visa will receive the I-20 Form from Stevenson after they submit their enrollment forms and enrollment fee. To learn more about the F-1 Student Visa you can visit the Student and Exchange Visitor Information System (SEVIS) website.
New enrolled students and their families are strongly encouraged to join us on April 27, 2013, for An Introduction to Stevenson for New Families. As part of the day's events, students will take a math placement test and those students seeking placement in a foreign language higher than level 1 will also take a foreigh language placement test. If you have completed your enrollment paperwork, you will receive an invitation from the Registrar's office to sign up for the placement test. The Registrar's office can be contacted by email. Additional information about academic placement for math, foreign language, honors, and advanced classes can be found on the New Student Information web page.
Upon submitting all necessary enrollment forms, families will receive an email that includes the Post Enrollment Checklist and a link to the health forms to be completed. In mid-May, families will receive an email about our Post Enrollment Questionnaire and directions for completing it online.
